VEHICLE-TWO: 2000 FORD PICKUP DRIVEN BY JUSTIN TYLER BERRY /SIC/ WHITE MALE, AGE 24, ADDRESS MIAMI, OK. HE WAS PRONOUNCED DEAD AT THE SCENE FROM MASSIVE INJURIES. TRANSPORTED BY PAUL THOMAS FUNERAL HOME TO TULSA MEDICAL EXAMINERS OFFICE.

WHAT HAPPENED

VEHICLE-1 WAS EASTBOUND. VEHICLE-2 WAS WESTBOUND. VEHICLE-1 WAS EASTBOUND AND CROSSED THE CENTER LINE FOR UNKNOWN REASON AND STRUCK VEHICLE-2 HEAD-ON.

DRIVER OF VEHICLE-2 WAS PINNED FROM 1953 HOURS TO 2053 HOURS FOR A TOTAL OF 1 HOUR. EXTRICATED BY MIAMI FIRE DEPARTMENT USING THE JAWS OF LIFE.
